Need to Request a Record?

Integra has designated specific staff to assist in fulfilling records requests.  We make every effort to respond to each request for records as promptly as possible, taking into account factors such as timing of request, complexity of request, volume of documents, and difficulty in identifying available records.  Each request is processed in the order in which it is received and we appreciate your understanding.  Records can be returned by either email or fax, and it’s our customer’s responsibility to choose the option of fax, if the requested records contain sensitive information.
